What is botulinum toxin?

It is a drug, derived from the bacterium Clostridium botulinium and is widely used in many medical conditions. In addition to its use to reduce facial wrinkles and fine lines, it is also used in muscle spasms (e.g. after a stroke), in migraines, in bladder diseases, in hyperhidrosis of the armpits etc. It is in form of powder which is diluted in normal saline.

How does it work?

This toxin inhibits muscle contraction by acting on neuromuscular junction (the point where the nerve instructs the muscle to contract). This makes the muscles loosen. In aesthetic medicine, this is the way to treat the dynamic (expression) wrinkles, which are caused by strong muscle contraction.

In which areas is it applied?

• Glabella.
• Crow’s feet.
• Forehead lines.
• Bunny lines.
• Nefertiti’s lift.
• Neck platysma.

How is it applied?

The treatment is applied by injecting the medication into the muscles of the treated areas using an ultra-fine needle. Because the infusion of the fluid creates a “tension” in the tissues, the patient feels a slight tingling, that lasts for a few seconds. The whole treatment lasts 10-15 minutes and the patient can immediately return to their activities. Wrinkle relaxers start “working” 24-72 hours later and rarely after 5 days. The final result is visible after 10- 15 days. It is when the patients come to the clinic for their touch up in case a mild correction is needed.

What should I be careful of afterwards?

The patient should not lie down, bend or rub the treated area for four hours after the treatment as there is a risk of drug diffusion. Exposure to high temperatures should also be avoided (e.g. using a hair dryer directly in the treated area, sauna, steam bath), because the toxin is degraded and the outcome of the treatment lasts for shorter time.

How often should the treatment be repeated?

It should be repeated every 4-6 months, that is 2-3 times a year.

Are there side effects?

• Immediately after the treatment there are small swellings (edema) (like mosquito bites) at the injection points that last approximately 30’. There’s also mild redness or bruises which recede quickly.
• Rarely can the patient feel headache or numbness which might last 15-20 days.
• Rare complications associated with drug diffusion which are reversible:
• Temporary fall of upper eyelid.
• Weakness or swelling of the lower eyelid.
• Inability to bend the neck.

Are there contraindications?

This treatment should not be performed to pregnant and lactating women. Also, to people suffering from muscle disorders (e.g. Gravis myasthenia).

At what age can one start the treatment?

There is not a certain age to start the treatment with wrinkle relaxers. The age of onset depends on each patient’s needs. The earlier one begins the treatment, the more they can avoid having permanent wrinkles on their face.

Do the patients lose their expression with this treatment?

The modern trend in medical aesthetics is the “natural look”. Thus, the patient treated with wrinkle relaxers by a trained, experienced dermatologist does not lose their expression, but looks fresher, more relaxed, radiant and healthy.

Is treatment with wrinkle relaxers alone enough to treat permanent, deep wrinkles?

Are there cases that wrinkle relaxers treatment is not working?

About 1% of the patients, who undergo this treatment, develop antibodies to the toxin, making subsequent sessions ineffective.

What if I decide not to continue my wrinkle relaxers treatment?

Expression wrinkles will gradually come back and look the same as before or even slightly better before starting treatment, without altering the physical features.
